The 5 DQF sections every carrier must maintain — and how to automate them

Interstate motor carriers must keep a complete Driver Qualification File (DQF) for each driver subject to 49 CFR Part 391. Missing or incomplete files are among the most common violations cited during FMCSA reviews. Below is a practical breakdown of the five core document groups safety teams should maintain — aligned with how auditors expect files to be organized — plus ways to reduce manual work without cutting corners.

1. Driver's application for employment

Part 391 requires a written application covering driving history, prior employment, accidents, and disqualifying offenses, as described in §391.21. The application is the anchor document for the rest of the file: later inquiries, MVRs, and annual reviews should be consistent with what the driver disclosed at hire.

Automation tip: Use structured digital applications with required fields, e-signatures, and validation so incomplete submissions cannot advance. Store the final signed version in the DQF with a timestamp.

2. Inquiry to previous employers (driving history)

For the prior three years of employment (and longer where §391.23 applies), carriers must document efforts to obtain safety performance history from prior DOT-regulated employers, including drug and alcohol violation history where applicable. Keep copies of inquiries and responses, or document good-faith efforts when a prior employer does not respond within required timeframes.

Automation tip: Track due dates for follow-ups, centralize responses, and generate an audit trail showing when each inquiry was sent and what was received.

3. Road test documentation or equivalent (e.g., CDL)

Under §§391.31–391.33, you must retain proof that the driver is qualified to operate the equipment they will drive — typically a road test certificate or an equivalent where permitted (for example, a valid CDL in circumstances where a road test is not required). Ensure the document matches the vehicle type and jurisdiction.

Automation tip: Link road-test or CDL records to the driver profile and trigger alerts when credentials approach expiration or when equipment class changes.

4. Annual Motor Vehicle Record (MVR) and annual review

§391.25 requires motor carriers to obtain an MVR at least once every twelve months from each state where the driver holds a CDL or permit, and to perform an annual review of the driver's driving record. The annual review must be documented and retained with the MVR.

Automation tip: Schedule recurring MVR pulls aligned to hire anniversaries, attach the annual review form automatically, and flag violations that require follow-up under your company policy.

5. Annual certification of violations

§391.27 requires drivers to provide a list of all violations of motor vehicle traffic laws and ordinances (other than parking) for the preceding twelve months, and carriers must add the signed certification to the DQF. This complements — but does not replace — the MVR and annual review.

Automation tip: Send annual reminders, capture electronic signatures, and retain prior years in chronological order for auditor review.
